Please include the following information in the e-mail :

1. The examples selected and quantity of each (please indicate example #).

2. Name, e-mail address, phone number. Please also include your address if you need your order shipped. You will receive an e-mail reply within 48 hours.

3. Desired form of payment. We accept Money Order, Check and Cash. In your confirmation e-mail you will be provided with the information needed to fill out a check or money order.
